Indications/Special comments:
Prophylaxis of embolization in rheumatic heart disease and atrial fibrillation; prophylaxis after insertion of prosthetic heart valve; prophylaxis and treatment of venous thrombosis and pulmonary embolism; transient ischaemic attacks.
Dosage Administration:
Adult: initially, 5 - 10mg daily for 2 - 5 days or until, the desired prothrombin activity is reached. Maintenance is determined by individual response and is usually 2 - 10mg daily (taken at the same time each day). Infants and Child: 0.05 - 0.34mg/kg/day; infants < 12 months of age may require doses at or near the high end of this range; consistent anticoagulation may be difficult to maintain in children < 5 years of age.
