    Dosage Administration: Status epilepticus: Slow IV injection or by IV infusion (with blood pressure and ECG monitoring): Adult: 15mg/kg at rate of not more than 50mg/minute, as a loading dose; maintenance doses of about 100mg by mouth or by slow IV injection should be given thereafter at intervals of 6 - 8 hours, monitored by measurement of plasma concentrations; rates and dose reduced according to weight. Child: 15mg/kg as loading dose at rate of 1mg/kg/minute (not exceeding 50mg/minute). Neonate: 15 - 20mg/kg as a loading dose at rate of 1 - 3mg/kg/minute.

    Indications/Special comments: Treatment of acute infections caused by susceptible strains of Pseudomonas aerginosa, reserved for life threatening infections caused by organisms resistant to preferred drugs i.e. Pseudomonal menigitis – intrathecal administration
    Dosage Administration: Infants < 2 years: IM: Up to 40,000 units/kg/day divided every 6 hours not usually recommended due to pain at injection site. IV: up to 40,000 units/kg/day divided every 12 hours; Intrathecal: 20,000 units per day for 3 - 4 days then 25,000 units every other day for at least 2 weeks after CSF cultures are negative and returns to normal limits. Children> 2years, Adolescence and adults: IM: 25,000 – 30,000 units/kg/day divided every 4 - 6 hours not routinely recommended because of pain at site; IV: 15,000 - 25,000 units/kg/day given evey 12 hours. Intrathecal: 50,000 units daily for 3 - 4 days then every other day for at least 2 weeks after CSF cultures are negative and CSF returns to normal limits. Total daily dose should not exceed 2,000,000 units. Bladder irrigation: Continuous irrigation or rinse in urinary bladder for up to 10 days using 20mg (equivalent of 200,000 units) added to 1L of normal saline. Usually no more than 1L of irrigant is used daily unless urine flow rate is high administration rate is adjusted to patients output. Topical irrigation or solution: 500,000 units per liter of normal saline maximum total daily dose should not exceed 2 million units in adults.

    Dosage Administration: Deep IM injection: Streptococcal pharyngitis; primary prophylaxis of rheumatic fever: Adult and Child over 30 Kg body-weight: 900mg as a single dose; Child under 30 Kg body – weight: 450 – 675mg as a single dose. Secondary prophylaxis of rheumatic fever: Adult and Child over 30 Kg body-weight: 900mg once every 3 – 4 weeks; Child under 30 Kg body-weight: 450mg once every 3 – 4 weeks. Early syphilis: Adult: 1.8g as a single dose, divided between 2 sites. Late syphilis: Adult: 1.8g divided between two sites, once weekly for 3 consecutive weeks.
