    Indications/Special comments: Prostate cancer in patients who cannot tolerate orchiectomy or oestrogen therapy (palliative), with flutamide and radiation therapy in the treatment of locally confined stage T2b–T4 (stage B2–C) prostate cancer. Advanced breast cancer in peri- and postmenopausal women (palliative). Endometriosis produces thinning of the endometrium before endometrial ablation for dysfunctional uterine bleeding.
    Dosage Administration: SC: Adults: 3.6mg every 4 wk or 10.8mg every 12 wk. Endometrial thinning: 1 or 2 depots given 4 wk apart; if 1 depot used, surgery is performed at 4 wk; if 2 depots used, surgery is performed 2 – 4 wk after 2nd depot.

    Indications/Special comments: Partial seizures (adjunct treatment) (immediate-release only). Postherpetic neuralgia. Restless legs syndrome.
    Dosage Administration: Epilepsy: Adults and Children >12 yr: 300mg 3 times daily initially. Titration may be continued until desired (range is 900 – 1800mg/day in 3 divided doses; doses should not be more than 12 hr apart). Doses up to 2400 – 3600mg/day have been well tolerated. Children =5 – 12 yr: 10 – 15mg/kg/day in 3 divided doses initially titrated upward over 3 days to 25 – 35mg/kg/day in 3 divided doses; dosage interval should not exceed 12 hr (doses up to 50mg/kg/day have been used). Children 3 – 4 yrs: 10 – 15mg/kg/day in 3 divided doses initially titrated upward over 3 days to 40mg/kg/day in 3 divided doses; dosage interval should not exceed 12 hr (doses up to 50 mg/kg/day have been used). Postherpetic Neuralgia: Adult: Immediate-release– 300mg once daily on first day, then 300mg 2 times daily on second day, then 300mg 3 times/day on day 3, may then be titrated upward as needed up to 600mg 3 times/day;

    Indications/Special comments: Biliary tract infection, bone and joint infection, meningitis, ventriculitis, urinary tract infection, peritonitis, bacterial septicemia.
    Dosage Administration: Adult: Antibacterial (systemic): IM or IV infusion: 1 - 1.7mg (base) per kg of body weight every eight hours for seven to ten days or more. Urinary tract infection (bacterial, uncomplicated): Adults (< 60kg body weight): 3mg (base) per kg of body weight once a day, or 1.5mg per kg of body weight every 12 hours. Adults (= 60kg of body weight): 160mg (base) once a day, or 80mg every 12 hours. Usual adult prescribing limit - up to 8mg (base) per kg of body weight daily in severe, life threatening infections. Child: Antibacterial (systemic): premature or full term neonates up to 1 week of age: 2.5mg (base) per kg of body weight every 12 or 24 hours for seven to ten days or more. Older neonates and infants: 2.5mg (base) per kg of body weight every 8 to 16 hours for 7 - 10 days or more. Child: 2 to 2.5mg (base) per kg of body weight every 8 hours for 7 - 10 days or more.
